Before becoming a patient of Bluegrass Internal Medicine, one must fill out a preliminary patient acceptance form. You will be notified either by mail or by a telephone call of your acceptance. Upon being accepted you will be mailed a New Patient Packet. Enclosed in the packet you will find an appointment card, a New Patient Information Sheet, a Health History form, an authorization for release of medical records, and a copy of our Privacy Policy. The Privacy Policy is required by the Health Information Portability and Accountability Act better known as HIPAA. You will also find enclosed a paper for you to sign stating that you received a copy of our Privacy Policy. In addition to the Privacy Policy, this form allows you to name a personal representative, someone close to you, which we may share your medical information with if you wish. This form will also allow you to choose how we can contact you if we need to.
Upon arrival to our office on the day of your appointment, the receptionist will ask you for these forms, please bring them with you. In addition, the receptionist will ask you for your insurance cards and driver’s license. The nurse will need you to bring all medications that you are taking, and any available medical records from your previous healthcare provider.
Once you have completed the new patient form you may fax it into our office at 270-713-0227 or drop it off at our practice, located at 3346 Professional Park Dr.
PARKING
Parking is available in the front of the building.
OFFICE HOURS
Our office is open from 8:00 A.M. to 5:00 P.M., Monday through Friday. The office hours for the telephone are 9:00 A.M. to 4:00 P.M. A physician is also on call 24 hours a day, seven days a week. We encourage patients to call between 9:00 A.M. and 4:00 P.M. unless it is an emergency.
PATIENT APPOINTMENTS
Patients are encouraged to keep their appointments. We would appreciate you telephoning the office 24 hours in advance if you must cancel your appointment so that we can schedule patients who request to be seen.
PAYMENT OF SERVICE
Our office participates with many managed care contracts, and we comply with the payment policies. It is the patient’s responsibility to know their insurance plans. Amounts not covered by the contracts are DUE AT TIME OF SERVICE, such as co-payments and deductibles. If we do not participate with your plan, or you do not have insurance and you are accepted as a new patient, full payment is expected at the time the service is rendered. At this time we accept cash or check, American Express, Discover, MasterCard and Visa.
